- Please read the full Fall 2022 Plan here to understand all COVID-19 expectations that you must follow. This includes:
- Student pre-arrival protocols, including information about the need to submit a pre-arrival testing attestation form or fully remote attestation form;
- Student proof of vaccination submission requirements;
- Fall 2022 COVID-19 testing information and schedules for students, faculty, and staff.

- Note: Each employee must submit a test sample August 30/31 or off-campus test result.

As federal, state, local, and SUNY health guidance changes due to the evolving COVID-19 pandemic, SUNY Poly’s rules will also evolve as the institution receives new guidance and/or regulations are promulgated.
