Student Vaccination Requirements + Guidance

Effective August 23, 2021, all SUNY Poly students are required to be vaccinated against COVID-19.

This mandate is in effect for any SUNY Poly student coming to any SUNY Poly campus or facility for any reason during the 2021-22 academic year, and went into effect upon the FDA granting final approval to the Pfizer COVID-19 vaccine on August 23, 2021.

SUNY System has provided strategic guidance to reopening safely this fall. As part of this guidance, SUNY’s system-wide approach to the vaccine requirement specifies a maximum 35-day grace period from the date of FDA approval, and the SUNY Poly deadlines mentioned below are designed to comply with this requirement. Careful adherence to these deadlines is important to ensure that your fall studies are uninterrupted.

If you have not already submitted your vaccination to the Wellness Center, please find more information on next steps according to the following options:

Already Received COVID Vaccine, but Have Not Submitted Proof of Vaccination

Have not yet received COVID vaccine, but will get vaccinated given the SUNY mandate

Any SUNY Poly student with a legitimate and documentable medical or other exemption as to why they are unable to receive the COVID-19 vaccine must submit an exemption request to the Wellness Center (medical) or Student Affairs (other) by Monday, August 30, 2021.  To request an exemption form, please email or Detailed information, including where applicable, a treating physician’s records (medical), must be provided.  Students whose exemption request is denied will have five calendar days after receiving the exemption denial to receive their first vaccine dose and provide documentation to

No additional remote learning options will be made available to students outside of the courses already scheduled. There will be no opportunity for students to request a change in course modality during the semester. Faculty may not make changes to their attendance policies to accommodate students who are not in compliance with the vaccination requirement. In addition, students should be aware that the SUNY tuition and fee refund policy remains in effect for any student who refuses to receive a vaccine and is not approved for an exemption.

Students who fail to meet the SUNY vaccination requirement or have not been approved for an exemption will not be allowed on any SUNY Poly campus or permitted to enter any SUNY Poly building and will be unenrolled from classes.

Please be reminded that all students returning to campus for the fall semester, who have not yet completed their full vaccination (two weeks after final dose) and submitted documentation to, are required to provide proof of a negative PCR within 72 hours of arrival to campus and will be required to participate in weekly surveillance testing.
