A ttention RF & State Employees:  2015 Flex Spending Account Claims Deadline is March 31, 2016

SUNY Poly Human Resources would like to remind NYS and Research Foundation employees who contributed to a Flex Spending Account (FSA) in 2015, that all unpaid claims must be submitted on or by March 31, 2016. If you fail to submit appropriate receipts for any unused funds by the March 31^st deadline date, you will forfeit such contributions and will not receive reimbursement or credit.

For NYS employees enrolled in an FSA, please send in reimbursement requests along with your receipts for eligible 2015 expenses by mail, fax, or submit online through www.myFBMC.com. For more information, log onto www.flexspend.ny.gov or call 1-800-358-7202.

For Research Foundation employees enrolled in an FSA, please submit reimbursement requests along with your receipts for eligible 2015 expenses online through CONEXIS at https://mybenefits.conexis.com/ or contact them at 1-866-279-8385.
