Effective January 1, 2014, the Empire Plan Prescription Drug Program is administered by CVS Caremark.

Letters were mailed to Empire Plan members from CVS Caremark and you must bring this letter to your pharmacy because it contains updated information your pharmacy will need to process your prescription billing records.  This letter is available online at http://www.caremark.com/portal/asset/NYSHIP_TempID_Empire.pdf
