Option Transfer 2015

If you wish to make any changes during this time, you must complete the NYS Health Insurance Transaction Form .

Please remember that if you are currently enrolled in the Opt-Out Program, you must complete a 2015 Opt Out Form  if you wish to have your opt out payments continue in 2015.

All forms and required documents must be received in the Utica Human Resources Office by January 16, 2015.
