UUP members who wish to change their Delta Dental plan election for 2017 may do so during the annual open enrollment period which is from November 1 – November 30.

There are two options under Delta Dental:

• Delta Dental PPO   https://www.deltadentalins.com/uup/pdfs/highlights_active_ppo.pdf
• Delta Dental DHMO  https://www.deltadentalins.com/uup/pdfs/highlights-dc-usa.pdf

If you want to change your plan, please complete the Enrollment Form  and return it to Audra Cornelius in the Utica HR office by the close of business Nov 30.

If you are changing from the PPO to the DHMO, you will also need to complete the DHMO Enrollment Application  and return that along with the enrollment form.
