What is the Educational Opportunity Program (EOP)?
SUNY Poly’s Educational Opportunity Program (EOP) provides access, academic support and financial aid to students who show promise for succeeding in college but who may not be eligible for regular admission.
Use this checklist as your guide to completing your EOP application and submitting your financial aid requirements.
Apply to SUNY Poly one of two ways:
- Apply online by submitting the SUNY Application. The SUNY Application allows you to apply to multiple SUNY schools with one application.
OR - Apply online by submitting the Common Application. The Common Application allows you to apply to over 500 participating schools with one application.
For a completed application submit:
- Official high school transcript(s)
- Two letters of recommendation from a school-based counselor or teacher
- Optional: SAT or ACT scores (College Board School Code: 0755, ACT Code: 2953) *
*SUNY Poly will not require SAT or ACT scores for students applying for the fall and spring 2023 enrollment terms. Upon application submission, applicants will be asked if they have test scores that they would like to be considered for admission.
Submit the EOP Financial Information form online one of two ways:
- If you apply using the SUNY Application, the SUNY Application Services Center (ASC) will direct you to EOP Financial Information Form. You can also find it here
OR - If you apply using the Common Application, you will need to obtain the EOP Financial Information Form on your own. It can be found here.
Mail the SUNY EOP Financial Documentation Cover Sheet + Required Documentation to SUNY:

To be considered for EOP admissions, the FAFSA must be completed.
Start here:
- Obtain a Federal Student Aid ID (FSA ID) from the U.S. Department of Education at fsaid.ed.gov. Parents of dependent students are also required to apply for a FSA ID.
- Apply for financial aid online via the FAFSA (Free Application for Federal Student Aid):
- Use your income tax information from 2 years prior to the academic year that you will enroll when completing the FAFSA.

- New York State Residents ONLY can apply for TAP as part of their online FAFSA application process.
