CSEA Performance Evaluations & Program Forms are due January 31, 2024.
The following information is provided to assist you in the completion of these forms.
2023 Performance Appraisal & Rating Section
This is for the Evaluation Period from January 1, 2023 to December 31, 2023.
Please complete:
• Section 1 – Employee Identification
• Section 2 – Performance Appraisal
• Section 4 – Supervisor’s Comments
• Section 5 – Performance Rating
• Section 6 – Review and Approval (Send to your supervisor for final approval)
• Section 7 – Employee Comments (The employee must be given the opportunity to add any comments before signing.)
2024 Performance Program Section
This Evaluation Period will be from January 1, 2024 to December 31, 2024.
Please complete:
• Section 1 - Employee Identification
• Section 2 - Performance Program (The employee should initial/date indicating receipt and review of their program.)
Both forms, 2023 Performance Appraisal and the 2024 Performance Program, are due to the Office of Human Resources for employee(s) under your supervision by January 31, 2024.
