ATTN: Adjunct Lecturers scheduled for Spring 2016: Human Resources Appointment Forms

Please be advised that all new and returning adjuncts are required to sign the Appointment Request form and appointment letter each semester.  These forms are required to process your new appointment in the payroll system.  We will NOT be able to generate a paycheck without successful completion of these documents.  The documents will be sent as attachments to your SUNY Poly email account at some point the week of December 7-11.  Please review the documents for accuracy, complete, and either scan and email the completed forms, drop hard copies off, or mail them to the Human Resources office.
