All students taking 12 credits or more and all Department of Nursing students regardless of credit hours are AUTOMATICALLY  enrolled and billed ($936./semester) for medical insurance.  If you already have medical insurance you must complete the on-line waiver once an academic year to remove the medical insurance fee.  If you waived it in the fall semester, then you will not be enrolled nor billed for it in the spring semester.

To provide those students an opportunity that forgot to complete the on-line waiver, the date has been extended to Friday, January 27, 2017. ABSOLUTELY NO WAIVERS WILL BE ACCEPTED AFTER THAT DATE.

ALWAYS check your tuition bill......... if $936. is posted by the medical insurance fee then you are enrolled and have the medical insurance.

Process:

• Complete the on-line waiver
• Staff will review and accept your waiver within 2 business
• The $936. medical insurance fee will be removed from your account
• You will receive an e-mail confirmation to your SUNY Poly e-mail account
