Immediate Action Is Required if you do not need the medical insurance.

ALL students registered for 12+ credits and all nursing students are automatically enrolled and billed ($452.) for SUNY Poly's designated medical insurance UNLESS you complete the electronic waiver.

If you completed the waiver for the fall 2015 semester it remains in effect for the spring 2016 semester.

Spring 2016 newly admitted students (12+ credits and all nursing students) are automatically enrolled and billed for SUNY Poly's designated medical insurance UNLESS you complete the electronic waiver.

If you have other medical insurance (under a parent, employer, etc.), submit the waiver on the web at (http://healthwaiver.sunyit.edu) insurance waiver immediately.

The date to waive the medical insurance has been extended to Friday, January 29th. Absolutely no waivers will be accepted after that date.

A PIN number and your medical insurance information (company name, address, etc.) is required to complete the waiver.
