If you supervise CSEA staff, please read below.

CSEA Performance Evaluation & Program Forms are due January 31, 2016.

In accordance with the agreement between CSEA and the State of New York, Article 40, Section 40.1, “An employee shall have a performance evaluation done annually.”  The Annual Performance Evaluation period for all CSEA employees is January 1 - December 31, and will remain the same regardless of changes in negotiating unit or reassignments.  Employees who are reassigned or change negotiating units and who have more than six months of service in title, will be evaluated upon their departure from the previously held position.

The following information is provided to assist you in completion of these forms.

2015 Performance Appraisal & Rating Section

Please complete:

  • Section 1 - Employee Identification
  • Section 2B - Performance Appraisal
  • Section 4 - Supervisor’s Comments
  • Section 5 - Performance Rating
  • Section 6 - Review and Approval (Send to your supervisor for final approval)
  • Section 7 - Employee Comments (The employee must be given the opportunity to add any comments.)

2016 Performance Program Section

Please complete:

  • Section 1 - Employee Identification
  • Section 2A - Performance Program (The employee should initial/date indicating receipt and review of their program.)

Both forms (2015 Performance Appraisal and the 2016 Performance Program) are now due in the Office of Human Resources for employee(s) under your supervision.
