Request Accommodations

Step 1: Complete the Request for Accommodations Form

• Complete the Request for Accommodation Form . The interactive form will expand based on your answers and can be used to request academic, dining, or housing accommodations.
• Housing requests often require additional information from a practitioner. If you are requesting housing accommodations or a support animal, plan to share the Accommodated Housing Application: Provider Form with your provider for completion.
• Provide as much detailed information as possible. The more information you share, the better we will be able to support you.
• You will receive a confirmation email with action items. Be sure to provide an email address you check regularly and carefully review the next steps needed to complete the process.

Step 2: Submit Supporting Information, if Available

Any information or documentation you can readily share can be uploaded with the Request for Accommodations Form. Otherwise, supporting information can be sent to sas@sunypoly.edu.

Step 3: Meet with a Member of Student Accessibility Services

This intake meeting is a critical part of the interactive process and is required to put accommodations in place.

• Review and understand the Differences Between High School and College .
• We may ask you to provide additional information if your Request for Accommodations Form does not include enough detail or if you have not provided supporting documentation.

Step 4: Finalize an Accommodation Plan

• If an accommodation plan is created, we will inform you of the next steps for using your accommodations.
• You can register with SAS even if you do not need or want to use an accommodation plan at this time. Please reach out to Student Accessibility Services or review our accommodations and services to learn how the office can support you.
• Not all accommodations will require you to meet with us.
