Requesting Accommodations through Student Accessibility Services
Students may self-identify or disclose their disability at any point in their academic journey. All student requests for accommodations must be routed through our process. If you are a faculty, staff, or guest on campus and need to request accommodations, please contact helpdesk-hr@sunypoly.edu.
Please follow the steps outlined below:
STEP 1: Complete the Request for Accommodations Form
- The Request for Accommodation Form is interactive and will expand based on answers. This form can be used for academic, dining, or housing.
- Housing requests often require additional information from a practitioner. Plan to share The Accommodated Housing Application: Provider Form with your provider for completion if you are requesting housing accommodations or a support animal.
- Provide as much detailed information as possible. The more information you share, the better we will be able to support you.
- You will receive a confirmation email with action items. It is important you provide an email you check often and review next steps to complete the process.
STEP 2: Submit supporting information (if available)
- Any information or documentation you can readily share can be uploaded with the Request for Accommodations Form. Otherwise, it can be sent to sas@sunypoly.edu.
STEP 3: Meet with a member of SAS:
This intake meeting is a critical part of the interactive process and required to put accommodations in place
- Review and understand the Differences Between High School and College.
- We may ask you to provide additional information if your Request for Accommodations Form is not detailed or if you have not provided supporting documentation.
STEP 4: Finalize a plan
- If an accommodation plan is created, we will inform you of the next steps in using your accommodations.
- You can register with SAS even if you do not need or want to use an accommodation plan at this time.
- Please reach out or review our Accommodations and Services to understand how our office can support you.
- Not all accommodations will require you to meet with us.
- Accommodations cannot be arranged retroactively (after the fact). Students are encouraged to disclose early
