This form may be used to report an alleged incident of sexual harassment, sexual violence, sexual misconduct, dating violence, domestic violence, and/or stalking to SUNY Poly’s Title IX Coordinators.

Students, staff, or faculty may use the form to file a report on their own behalf or on behalf of a member of the SUNY Poly campus community.
