Central New York Health Home Network (CNYHHN)
CNYHHN, Inc. is one of 32 Lead Health Homes in New York State, designated by the NYS Department of Health. As lead Health Home in Oneida County and Central New York, CNYHHN serves an average of 5,200 Medicaid enrolled members annually and provides services within 10 counties. Across the state, over 180,000 high-risk, high-need adults and children are served annually through Health Homes. Additionally, CNYHHN has a network of 50 partnerships over a 10 county region. Research has found that Health Homes achieved a 27 percent reduction in healthcare costs for its enrolled members through care management and coordination.
CNYHHN is unique in that it is the only Lead Heath Home statewide that expanded its reach of serving vulnerable populations by creating an integrated care system with its collaborative partnering agencies through a hub and spoke model. In addition to its array of Community Network Partners, CNYHHN built school partnerships serving over 20,000 students through the Connected Community Schools Initiative, which includes13 School Districts and 53 school buildings. They also act as a co-leader with Rome Alliance for Education.
Our mission for the Connected Community Schools Initiative is to ensure that every student and family’s basic needs are met so they are prepared and capable to be successful in their education and everyday lives. Often if the students' needs go unmet, it will act as a barrier to their academic, social, and emotional success.
The Connected Community Schools Initiative connects students and families to appropriate services based on identified needs from the school faculty/staff or family members. Through the partnership with SUNY Polytechnic Institute, Daneli Partners, and the Herkimer County school districts, the team will build upon existing practices to provide support and case management. The dedicated Student & Family Advocate for the Liberty Partnership Program will provide supportive services by developing a personalized learning plan, building a positive and supportive relationship, and bridging the supports from school to home for the family.
With decades of combined experience, the Daneli Partners team consists of practitioners and leaders who are personally driven and impassioned to reenergize leaders by serving the growth and development needs that are specific to the challenges that exist in their individual environments.
Utilizing Clifton Strengths, students will learn more about their innate talents through self-discovery. The assessment will help them harness their potential and become the leaders they were meant to be.
