Health Requirements

To protect your health, the health of the campus community and to be compliant with the New York State (NYS) Public Health Laws, all students (unless 100% online, see details below) MUST provide the mandatory health requirements.  Students who do not meet New York State MMR and Meningitis requirements within 30 days of the start of the semester will be dropped from their classes for non-compliance.

New York State Public Health Law requires that all students registered for six or more credits show proof of immunity to measles, mumps, and rubella (MMR) and either have the meningitis vaccination (ACWY) within 5 years or Meningitis B two or three dose series, or decline it.  Persons born prior to January 1, 1957, are exempt from the MMR requirement.
Students must provide the following health documentation:

Measles, Mumps & Rubella (MMR)

  • Measles Documentation Required: Two dates of immunization on or after the first birthday or date and results of positive measles titer.
  • Mumps Documentation Required: Date of immunization on or after the first birthday or date and results of positive mumps titer.
  • Rubella Documentation Required: Date of immunization on or after the first birthday or date and results of positive rubella titer.

Meningitis

Meningitis Documentation Required:

  • Meningitis (ACWY) – One date of immunization within the last 5 years or Meningitis B two or three dose series

 OR

  • Completed and signed Meningococcal Information Response Form indicating acknowledgement of meningococcal disease risks and refusal of meningococcal meningitis immunization signed by the student (or student’s parent/guardian if under 18 years old).

The Meningococcal Information Response Form must be returned if vaccine documentation by a healthcare professional is not provided.

COVID-19 Vaccine Requirement

SUNY Announces End of the Emergency COVID-19 Vaccine Mandate:
Beginning Summer 2023, all SUNY Poly students with an on-campus presence are strongly encouraged to be fully vaccinated against COVID-19 pursuant to SUNY’s guidance. COVID vaccination will no longer be mandatory for attendance. Additionally, religious and medical exemptions no longer apply.

Health History and Physical Examination

All students taking 12 or more credits must provide a physical examination (with complete review of systems) completed and signed or stamped within the last 2 years by either a physician, nurse practitioner or physician’s assistant. A physical exam is required for all part-time students that utilize services at the Wellness Center and residential students. Any student under the age of 18 years old must have the second page of the Health History and Physical form signed by a parent or legal guardian to provide authorization for medical care at the Wellness Center.

There are no health requirements for students who are fully online (unless living on campus).

If you are a student taking 6 or more credits of a hybrid course, you may need to be on campus and you must comply with health requirements. These students should refer to the "I am part-time on campus (6 credits or more)" dropdown on this webpage.

Certain student populations must follow specific health requirements:

Nursing Department & Health Information Management students are required to send New York State Public health requirements and most recent physical exam to the Wellness Center.

Students are required to upload New York State Public health requirements (MMR and Meningitis documentation) and most recent physical exam to the Wellness Center using the Wildcat Wellness Student portal at myhealth.sunypoly.edu.

Health records are not automatically forwarded with your academic transcript. You must submit your health records to the SUNY Poly Wellness Center. See details below.

Tips for obtaining a copy of your health records for submission to SUNY Poly:

  • Ask your healthcare provider to provide you a copy of your recent physical and/or immunizations.
  • Ask your former high school or college for your immunization documents (as these records must be kept for at least seven years).
  • If you are serving or have served in the military, it is not uncommon for a delay in receiving your health information from the military. Please contact our office and we will work with you to meet the health requirements.

Where can I get the MMR or Meningitis Vaccine?
The Poly Wellness Center does not offer the MMR or Meningitis Vaccine at this time. You can receive these vaccines at the County Health Department or a local pharmacy. Below are a few to select from (though you can go anywhere you choose). Please check with your insurance company regarding any cost associated with the vaccine.

Please submit all required health documents to the Wildcat Wellness Student Portal by August 1 (for Fall semester) and January 1 (for Spring semester):

  • Log into myhealth.sunypoly.edu and follow the instructions below:
  • Log in with your SUNY Poly email and password
  • Enter your birthdate (MM/DD/YYYY) then click Proceed.
  • Click on the "Forms" tab on the left side bar and click the green "Update" box to upload your documents and/or download required forms.
